The Life Shape Program
is a multidisciplinary surgical weight loss program that offers the Laparoscopic Adjustable Gastric Band (LAP-BAND®). Our mission is to improve the health of our patients by performing the LAP-BAND® surgery with the highest degree of safety possible, and to maximize weight loss results with a comprehensive long term multidisciplinary weight loss program.

Components of Our Program

Our first goal is to provide patients with the information they need to see if the LAP-BAND® is right for them. To achieve this goal we provide patients with an information packet about the LAP-BAND®.

We ask that patients attend one of our patient information seminars. The seminar includes the opportunity to meet some of our former patients.

If patients decide they wish to be considered for LAP-BAND® surgery, the following evaluations are required before surgery.

• Medical Evaluation by the patient's primary physician. (please do not schedule this visit until you have received our preoperative packet that includes a form for your doctor to fill out)
• Preoperative physical fitness assessment with Certified Personal Trainer
• Preoperative psychological evaluation
• Preoperative nutrition consult
• Online computer registration at the Melbourne Surgery Center Site
• Preoperative Anesthesia evaluation. by a member of the Brevard Anesthesia Group.
• Preoperative appointment with LifeShape's nurse aftercare coordinator
  
• Completion of the EMMI® on-line educational program
  
• Preoperative evaluation with Dr. Fusco

Please note that even though patients are given target surgery dates prior to completion of all the listed appointments, the final clearance to have surgery is NOT complete until ALL visits are completed.
